I'm guessing nose job - it's not curved like that now, and as he suggested cheek implants (maybe...), and ear pinning (definetly 100%).

His chin is huge...it looks like a wooden puppet's chin from the side...it just doesn't end.

Except for the ears I almost like the before pic better. Looks more natural..now he almost looks like a cyborg-just not quite human like-too much like an action figure or something-especially in that picture.

I think he did something to his eyes or eyebrows, too cuz in the old pic, his eyes look kinda weird, like a bit wonky or something.
