Katie looks drugged, as usual, but I can't really tell what (if anything) Tom has had done. Maybe botox, because of the marble forehead, but other than that, he actually looks kind of rough to me, at least in this picture.
Suri merely looks like she's glancing down, at possibly something she dropped or just noticed.
