It's certainly possible to get a nasty infection from a tattoo, especially a large one. Especially if a person was a procrastinator about treatment.
But I have serious doubts that was the case here. He was a user and supposedly in and out of rehab for heroin. If he'd been clean for a spell and then fell off the wagon it's actually very easy to OD with that drug.

I'm not saying I believe this story, but could death by tattoo be possible if the person was already in really bad health? His body must've been wrecked if he was an addict.

Not death by tattoo per se. But death by infection. And infections are not uncommon. A tattoo is basically an open wound until healed.

You must provide a SOURCE when posting in this forum. You had to have heard this somewhere or seen it reported right? If it is not sourced it is a violation. A source should be provided or the thread should be removed. Otherwise anyone could just make up any old thing and post it. (not saying that was done in this case of course).just sayin'
This does just sounds like a rumor to me IMHO.

From TMZ this morning:
TMZ has confirmed that Brad Renfro had a young son who lives in Japan with his mom -- and the family is trying to keep details about the boy under wraps.
TMZ spoke with Renfro's dad, Mark, and his step-mom, Kim, who would not reveal their grandson's name or age in order to protect his privacy. They placed an obit in a local paper that simply listed his name as "Y Renfro."
Kim told us, "We did expect this because people didn't know he had a son. But we're very thankful Brad got to see his son before he died."
Brad was recently in Tennessee with the boy, spending time with the family.
A source close to Renfro, who spent a lot of time with the actor in the months before his death, tells TMZ she didn't know he had a child.

It is rumour. According to the L.A. Times and other news outlets the autopsy doesn't even take place until Thrusday the 24th.

You're supposed to put a medical salve on tattoos to keep the skin clean while it heals. He wouldn't have been able to reach his back to do it himself, should have had a friend do it for him.

I agree, cause of death at this point is rumor/hearsay. But, the autopsy was actually done last Friday (was originally scheduled for last Thursday)
Renfro autopsy planned for Thursday - Los Angeles Times
They had his memorial service/funeral yesterday.
