You can get implants inserted under the muscle or on top of the muscle. You can also get round ones or tear drop shaped ones. Ones that are placed under the muscle and tear-drop shape definitely look more natural and better. (No, I don't have them, but I did some medical writing work on plastic surgery once.)

I asked the Mr's cousin about this (I just had to, they were staring me in the face) and she said that she spent a lot of money on them and she wanted people to know that she had spent a lot of money. To her it was a sign of wealth as much as the designer logos on her clothing. Probably one of the weirdest funeral conversations ever. Definitely one of the most inappropriate funeral outfits ever.
