What Roaseanne and her family did in that episode, though... it didn't necessarily look that smart to me. They were out in the open, and anything could've come in and hurt or killed them. But then again, it's a show, so they did it for the drama, but you don't want to be out in the open, during a tornado, nor do you wanna be in a car, or anywhere outside. If you're outside, hide in a ditch.

Of course if it is a really bad (EF5) tornado, then even getting in the basement may not help. In the Parkersburg Iowa tornado recently, of the six people killed, three were actually killed while hiding in their basements-

And they were all elderly.
